Collect specimen into vials labeled with patient MRN, name, and DOB in black inkProviders will need to fill out a LabCorp requisition form. These will be available from your AA.Circle the Provider’s name and NPI, listed on the left hand sideCheck Billing info-Medicare and/or Medicaid on left hand sideFill out insurance details or print face sheet or send request with note to AA asking for copy of face sheet to be sent with labsPatient demographics and lab collection info - name, sex, dob, collection time, collection date, patient addressPatient MRN in Patient’s ID # box (important to ensuring results get into Epic)Sign at Physician Authorized Signature x_____________Write in ICD-10 codes for test/s being orderedConfirm our account number 31133310 is printed above the patient nameEach requisition form has stickers with the account number that must be placed on the tube/specimen, in addition please write patient’s name and DOB on each tubePlace the labelled specimen in a lab specimen bag (in the storage closet) and insert the filled-out LabCorp requisition form in the pocketProvider places specimen and requisition form in LabCorp box located in lobby of CAM behind security guard or respective SFR site in the front office with clerkProviders then send EPIC LabCorp request to AA – give the carbon copy of the form to your AA to be scanned into the chart AA will call for pickup (800-631-5250), press option 2, then press option 3 and speak with a live representative. Collect specimen into ThinPrep Vial labeled with patient MRN, name, and DOB in black inkWhen back in the office: a requisition order will print out when order is signedPlace the requisition order with the specimen in a hazard bagTo re-print requisition order go to Chart Review –> Path/Cyto tab –> double click on the order –> Scroll down to click on “Lab and Collection Information” –> print requisition formTake specimens to 15th floor of Annenberg and drop off at Pathology (should have someone there to collect samples 24x7)

Insert speculum and visualize the cervixBroom: Insert the central bristles of the broom into the endocervical canal deep enough to allow the shorter bristles to fully contact the ectocervix. Push gently, and rotate the broom in a clockwise direction five times.Spatula/Brush: With the long arm of the spatula towards the os, complete one 360 degree turn maintaining contact with the ectocervix. Insert the brush into the endocervical canal until the bristles just disappear. Rotate clockwise 180 degrees (one half turn).Rinse the broom or spatula/brush as quickly as possible into the ThinPrep solution vial by pushing the broom into the bottom of the vial 10 times, forcing the bristles apart.Swirl the vigorously to further release material and then discard the collection device. Do not leave the collection device in the solution. #

Dermatopathology

Collect specimen into container with fixative (usually 10% buffered formalin).
