Looking Back
- Started in 1995 to care for East Harlem patients
- Merged with Geri Dept. home visit program in 1997
- Chelsea-Village House Call Program joined in 2010
- St. Luke’s-Roosevelt Hospital House Call Program joined in 2014
Looking Ahead
- Care for about 1,200 patients with over 6,000 home visits annually
- Home visits by 20 MDs and 6 NPs, and at times one of 6 SWs
- Office staff of 7 administrators and 2 RNs for triage and new referrals
- Call service 24/7 days for urgent matters
