Visiting Doctors Work Flow 11/1/17
NEW PATIENT
Providers:
- Document the visit
- Document the INITIAL H/P FLOW SHEET
- Change the PCP in EPIC - go to snapshot and double click on the care teams tab (or any of the multiple ways to change provider.)
- Add patient to your “patient list” in EPIC
- Route chart to your AA for next appointment.
- Route chart to your AA and Courtney Williams and indicate in the routing comments “new patient.”
- Put documentation to be scanned in 2 batches – one batch for HCP/DNR etc and one for everything else.
AA’s:
- Will document in IDX/EPIC that patient is “Active.”
- Will document in IDX/EPIC the PCP.
- Document the PATIENT STATUS TRACKING TOOL VISITING DOCTORS flow sheet – indicate that the patient is now active and indicate the date the patient became active
PLACE PATIENT ON HOLD OR TAKE THEM OFF HOLD
Providers (MD or NP)
- Send a staff message to your AA to put patient on hold or take them off hold.
AA’s:
- Will change the patient’s status as appropriate in IDX.
- AAs & SWs keep a standing list of all patients placed on HOLD for 3 months; if exceeds 3months patients are discharged from the program
DISCHARGE FROM THE PROGRAM
Providers(MD or NP):
- Fill out the FPA VISITING DOCTORS DISCHARGE FORM PART 1 PROVIDERS in EPIC.
- Route the chart to your AA
- If patient is not deceased but discharged from the program – remove yourself as the PCP in EPIC.
- Remove the patient from your “patient list” in Epic
- Deactivate patient from MyChart – go to “More” tab (bottom left on EPIC screen), click “MyChart Utilities”, then click “MyChart Administration” finally click “Deactivate Patient”
AA’s:
- Complete the DISCHARGE FORM PART 2 and in EPIC change the patients status in the PATIENT STATUS TRACKING TOOL VISITING DOCTORS indicating the reason of discharge, date and why.
- AAs completes Discharge forms, cancel all future visits and standing labs orders. Discharges patients in EPIC/IDX and routes back to Providers to notify complete
- Mark patient as VD-inactive in IDX if necessary. Mark patient as deceased in IDX if appropriate
