New Patient/Discharge Workflow

Visiting Doctors Work Flow 11/1/17

NEW PATIENT

Providers:

  1. Document the visit
  2. Document the INITIAL H/P FLOW SHEET
  3. Change the PCP in EPIC  - go to snapshot and double click on the care teams tab (or any of the multiple ways to change provider.)
  4. Add patient to your “patient list” in EPIC
  5. Route chart to your AA for next appointment.
  6. Route chart to your AA and Courtney Williams and indicate in the routing comments “new patient.”
  7. Put documentation to be scanned in 2 batches – one batch for HCP/DNR etc and one for everything else.

AA’s:

  1. Will document in IDX/EPIC that patient is “Active.”
  2. Will document in IDX/EPIC the PCP.
  3. Document the PATIENT STATUS TRACKING TOOL VISITING DOCTORS flow sheet – indicate that the patient is now active and indicate the date the patient became active

PLACE PATIENT ON HOLD OR TAKE THEM OFF HOLD

Providers (MD or NP)

  1. Send a staff message to your AA to put patient on hold or take them off hold.

AA’s:

  1. Will change the patient’s status as appropriate in IDX.
  2. AAs & SWs keep a standing list of all patients placed on HOLD for 3 months; if exceeds 3months patients are discharged from the program

DISCHARGE FROM THE PROGRAM

Providers(MD or NP):

  1. Fill out the FPA VISITING DOCTORS DISCHARGE FORM PART 1 PROVIDERS in EPIC.
  2. Route the chart to your AA
  3. If patient is not deceased but discharged from the program – remove yourself as the PCP in EPIC.
  4. Remove the patient from your “patient list” in Epic
  5. Deactivate patient from MyChart – go to “More” tab (bottom left on EPIC screen), click “MyChart Utilities”, then click “MyChart Administration” finally click “Deactivate Patient”

AA’s:

  1. Complete the DISCHARGE FORM PART 2 and in EPIC change the patients status in the PATIENT STATUS TRACKING TOOL VISITING DOCTORS indicating the reason of discharge, date and why.
  2. AAs completes Discharge forms, cancel all future visits and standing labs orders. Discharges patients in EPIC/IDX and routes back to Providers to notify complete
  3. Mark patient as VD-inactive in IDX if necessary. Mark patient as deceased in IDX if appropriate
