Summary: SPOP helps older patients obtain in-home mental health services. Eligibility depends on medical necessity, and high motivation to engage in services. Severe dementia will disqualify patients. Mild to moderate dementia required further evaluation before acceptance into the program. * 40% of their patients are homebound

Eligibility
- 55 years of age IN MANHATTAN
- Zones 2,3,9,10,11
Disqualifying factors:
- HOSPICE Patients
- Severe dementia
Services
- Bereavement support program as well as other MH services
- 45 min session weekly-- therapist
- Psychiatrist, SW will do intake and psychiatrist will do monthly medication mgmt.
- 6 mos to several years
- Warm hand-off approach*
- Work with VNS
* Joint visit -- PCP no billing on the day of the warm hand-off
Typical Diagnoses
- Anxiety or depression
- Adjustment disorders-- spousal loss etc
- Schizophrenia -- serious and persistent mental illnesses
- Substance abuse disorders-- harm reduction focus
- work on improving relationships
- helping to reduce use
- Caregiver stress-- as long as the person is greater than 55-- can be seen in one of the clinics.
